Speaker Request Form (to be used if your invitation includes ANY speaking role at your event)
If you are interested in inviting a Council member, our Executive Director, or one of our staff to speak at your event, please fill out the Speaker Request Form. Requests for PCFSN participation should be submitted at least eight (8) weeks prior to your event to allow ample time for processing and scheduling of your request.
Event Request Form (to be used only if your invitation does NOT include any speaking role at your event)
If you are interested in inviting a Council member, our Executive Director, or one of our staff to attend, exhibit, lead a physical activity or participate in your event in a role other than as a speaker, please fill out the Event Request Form. Requests for PCFSN participation should be submitted at least eight (8) weeks prior to your event to allow ample time for processing and scheduling of your request.
Once you fill out our Speaker or Event Request Form and click the "Submit" button, this should automatically generate a response e-mail which you should receive within a few hours after submitting your form. This e-mail will also contain a copy of your submitted form for your records. If you DO NOT receive this automatically generated e-mail, then your form did not submit for some reason and you should submit your form again. If you continue to experience problems submitting your form please contact our Deputy Director of Communications, Joey King, at email@example.com to report this problem, and to request that we e-mail you a PDF version of the form that you can submit as a alternate means of requesting PCFSN participation at your event. We apologize for any inconvenience this may cause.
